1. USES AND DISCLOSURES

Below is a description of how Keeley Home Care LLC is permitted to use information for service coordination, payment, and operations:

• Service Coordination: Disclosures to facilitate the non-skilled Service Plan (Plan of Care), such as sharing scheduling information with the family or your designated representatives.
• Payment: Submission of documentation to Long-Term Care (LTC) insurance providers or other entities responsible for paying for your non-skilled services.
• Agency Operations: Internal use to maintain the high standards of our Home Care Professionals and our RN-led non-skilled quality oversight for the family.

2. PERMITTED USES WITHOUT AUTHORIZATION

We are permitted or required by law to disclose Personal Health Information (PHI) without written consent for public health activities, reporting abuse or neglect, and health oversight activities as required by the State of Michigan.

3. COORDINATION OF SERVICES

The agency may contact the family to coordinate visits and provide updates regarding the non-skilled Service Plan.

4. INDIVIDUAL RIGHTS

You have the right to request restrictions on certain uses of your information. You have the right to inspect and copy your agency service records. We will fulfill written requests for records within 15 days.

5. OUR LEGAL DUTIES

Keeley Home Care LLC is required by law to maintain the privacy of your family's information and provide you with this Notice of our professional privacy standards.
